Consultation Conundrums solved! Zwivel is here with free, secure, online cosmetic surgery consultations.

Gary D. Breslow, MD had a genius idea to tackle the “consultation conundrum” – the apparently universal and yet seemingly unresolvable phenomenon where a plastic surgeon has to actually waste time going through a consultation with an unqualified patient to find out that it was, in fact, a waste of time to go through the consultation in the first place.

Dr. Breslow had an idea while discussing this with some colleagues.

“The consensus was that this affected everyone, unless your conversion rate was 100%, which even for the busiest plastic surgeons it never was.  On average more than 6 out of 10 patients who go in for a consultation for a cosmetic procedure with a cosmetic doctor never have a procedure with that doctor.  And, popular strategies like charging for consultations didn’t work either, as that effectively created a barrier and diverted serious potential cosmetic patients to the offices of those cosmetic doctors who did not charge a consultation fee. And, when you think about it, the plastic surgeon’s time is worth more than the consultation fee charged.

Then one day in January, 2012 after a day filled with back-to-back-to-back wasteful consultations, I boarded a plane for an annual father-son ski trip, and vented to my close friend about the frustrating inefficiency of the cosmetic consultation process.  For 4 hours we sat there, going back and forth, working through a possible solution.  By the time we landed in Utah, Zwivel was born.

Zwivel is a free, user-friendly, interactive online consultation tool that plastic surgeons can place on their own website enabling them to interact with, and prescreen for, potential cosmetic patients who visit their website prior to a full, in-office consultation.  Patients who visit the doctor’s website can click on a “Start Your Consultation Now” widget, then select their concerns and desired procedures, upload photos and record video of themselves, and answer some basic questions about their medical history, budget and time frame for having a cosmetic procedure.  The plastic surgeon can then review the information and respond with their cosmetic procedure recommendations and estimated fees, and can record their own video.  The patient can then review what the plastic surgeon says and decide if they want to come in for a full, in-office consultation.   The entire process is completely HIPAA/HITECH compliant, private and secure, and one-to-one between each doctor and each patient.

Swivel

While the version currently in use is free, ZwivelPremiumTM, which is scheduled to launch in July, charges doctors a monthly subscription fee for use of its enhanced features, which include the ability to filter out for only those procedures they perform, the option to include their staff to participate in their “Replies”, and the ability to customize, save, and re-use previous “Replies” and upload supporting videos and before and after images or other testimonials that are specific to the requested procedure.

Also with ZwivelPremiumTM, member doctors are automatically listed on the Zwivel.com’s  ZwivelSearchTMsite where patients can search for and review physicians’ profiles, rankings and reviews, and then send their information to multiple doctors at once, helping them in their decision process to choose the physician that is right for them and their desired procedure.  A nationwide multimedia public relations and advertising campaign is being launched to educate patients on Zwivel and how simple it can be to find out which doctor and which cosmetic procedure is the right one for them.

As the old adage says, time is money. Remember, 90% of all cosmetic patients are women – most of whom have jobs, children or both, and don’t have time to go to multiple plastic surgeons to find out which doctor and which procedure is right for them.

The consensus was that this affected everyone, unless your conversion rate was 100%, which even for the busiest plastic surgeons it never was.

Your Body Clock Effects Eveything: WhatYou Need to Know Now

July 1, 2015

Even when we’re not at work we’re on the clock–a biological one that is. Feeling bloated? Chronically fatigued? Messing up one of your “body clocks” is like a violinist playing a different tune than the rest of the orchestra—disastrous to the overall performance, and in the body it’s disastrous to long-term health.

In their new book, Strong Medicine (DragonDoor Publications, April 2015) Dr. Chris Hardy, D.O. and Marty Gallagher break down our body’s “master clock” to explain which parts are working to their maximum degree at which times in the day.

Where exactly are our master clocks are ticking from?

A master clock resides deep inside the brain. It governs cycles of sleeping, waking, eating, fasting, body temperature, and controls the ebb and flow of the various hormones that control our metabolism. This clock is not constructed of metallic gears, it is a molecular clock made of genes and protein located within specific nerve cells of the hypothalamus. Recall that the hypothalamus is the control center of the brain that responds to signals from the environment. It controls many of the body’s processes, so it makes sense that the hypothalamus houses the master clock.

What are the “9 body clocks”?

  1. The adrenal gland’s clock helps maintain the timed secretion of cortisol in the early morning to prepare for activity by increasing glucose release from the liver. It reduces cortisol levels in the evening and through the night to promote rest and regeneration.
  2. The heart clock helps prepare the heart for daytime by making it more responsive to the demands of physical activity with heart rate and strength of contraction. At night, the clock allows the heart to be less responsive during sleep.
  3. The liver clock helps control the “glucose banker” so that the liver can store glucose during daytime activity and meals, then release and make glucose at night during fasting to supply the brain.
  4. The pancreas clock sets insulin secretion at the highest during daytime activity to allow for energy storage. At night, insulin secretion is set low to ensure the brain receives a steady supply of glucose for regeneration.
  5. The fat clock controls the release of the hormones adiponectin and leptin. Adiponectin is higher during daytime activity to increase insulin sensitivity, allowing the storage of nutrients. Leptin is higher at night, delivering the “stop eating” signal necessary for fasting during regeneration.
  6. The muscle clock helps prepare our muscles for optimum performance during the day. The clock sets a metabolic rhythm, making it easier for muscle to burn glucose during the day, then using fat for energy at night.
  7. The immune clock has the immune system on alert for threats such as bacteria and viruses in the daytime, then at night the adaptive assassins create “immune memories” for the threats encountered during the day.
  8. The kidney clock helps control fluid and electrolyte balance in the body. To support daytime activity, blood pressure is increased, then at night there’s a 10-20% drop in blood pressure during regeneration.
  9. The gut clock regulates gastrointestinal function to increase digestion, absorption of food, and motility (movement of digested food) during the day. It signals for repair of the gut lining at night when digestive processes are minimized.

At what time of day are our “body clocks” most effective? When is our “Immune Clock” fighting bacteria and viruses most effectively? When is our Adipose (fat) Clock signaling our bodies to store the most nutrients?

During daylight hours, hormone levels are controlled by the master clock in the hypothalamus to support increase  physical activity. During the onset of night, hormone levels are controlled by the master clock in the hypothalamus to support repair and regeneration. The immune system works the best during the day to fight off bacteria and viruses directly and then generates “memory” immune cells at night (T-cells and B-cells) to promote future immunity to specific pathogens encountered during the day. The fat clock controls the release of the hormones adiponectin and leptin. Adiponectin is higher during daytime activity to increase insulin sensitivity, allowing the storage of nutrients. Leptin is higher at night, delivering the “stop eating” signal necessary for fasting during regeneration.

Why having a desynchronized body clock can lead to chronic illness– and how to reverse the chronic consequences.

Peripheral body “organ clocks” are set by the “master clock” in the hypothalamus. Each organ clock controls organ functions during activity and regeneration modes. The master clock regulates all of our major organs including the liver, lungs, heart, kidneys, gastrointestinal tract, and our muscles. They are crucial in ensuring the organ systems work together in a synchronized manner. Desynchronized (broken) clocks lead to chronic disease.

Screen Shot 2015-05-21 at 1.09.21 PMI would say that anything worth doing well is worth procrastinating.  This applies for everything from term papers in college, to parking tickets in the real world and of course skin care. Procrastination in skin care looks a little like this:  Monday night skincare routine, fall asleep in kid’s bed with dirty face.  Tuesday morning skincare routine, wake up in different kid’s bed then exercise.  Tuesday night skincare routine, fall asleep in dog’s bed.  Wednesday morning skincare routine, wake up in kid’s bed then carpool.  Wednesday night skincare routine, “shit I should hang out with my husband and my skin looks hideous!”  So when you rely on last minute quick fixes to undo days of damage, they better be good.

The absolute best for the most reasonable price point were both developed my Dimitri James, world renowned make up artist and life changer.  The first is  DermAppeal ($35.00).  It’s a Thermo-Enzymatic Microdermabrasion treatment with VC5 Crystal Technology that you only need to apply for a minute or so twice a week and it literally transforms your sensitive skin into a face that has been gently exfoliated daily (ha!). It also has this cool minty smell that opens up my constantly clogged sinuses (Jewish girl curse) and makes the entire one minute process feel like a quick trip to the eucalyptus room at your favorite totally over-priced spa.

Part 2 of operation “holy shit why did I not wash my face for days?” is aptly called “Take The Weekend Off.”  It’s a thick gooey creme that immediately absorbs into the skin so you don’t even know it’s there.   It’s completely translucent so you can read your children to bed without scaring them or have sex with your husband without repulsing him.  It’s the ultimate in multi-tasking.  You can literally connect with your kids, save your marriage, get a good night’s sleep,  all while cleaning out impurities in your skin and doing away with fine lines.

You’re welcome.
